Sponsorship

As part of Behind the Smiles Foundation's mission, we will award two applicants per year with three(3) free therapy sessions. The criteria and sponsorship eligibility form are listed below.

Please note that we are not licensed clinicians. This is a pre-screening form and the individual completing this form must be the interested party.

We respect your privacy and will ensure that any information you share with us is kept confidential and not used for any other purposes.

**Your sponsorship will lapse 30 days following approval if you fail to choose a therapist or schedule an appointment.

Criteria

1. You must be between the ages of 18 -30
